The key idea is recognizing the signs that point to air trapping under pressure in the chest after trauma. In a tension pneumothorax, air accumulates in the pleural space, collapsing the lung on the injured side and squeezing the mediastinal structures, which sharply reduces venous return to the heart. That combination produces two telltale findings: the lung on the affected side has markedly diminished or absent breath sounds from the collapse, and neck veins become distended (JVD) from the impaired venous return. Together, absent breath sounds on the injured side plus JVD strongly indicate a tension pneumothorax and require immediate intervention.

Lungs that are clear on the affected side argue against a pneumothorax. Hypotension with bradycardia is not typical here, since tachycardia is more common due to sympathetic response. A widened pulse pressure with loud breath sounds does not fit the pressure dynamics of a tension pneumothorax.
